In residential suites that offer a true feeling of home, we deliver extensive rehabilitation services to patients of all ages. Our internal nurse case manager, who serves as a patient advocate, is the link between the patient, their family, physician and our own nursing staff. And our comprehensive home transition program helps patients regain skills of daily living, from cooking and laundry to dressing and bathing, to foster independence before they return home. Social services and family involvement are integrated into the program to maximize successes and minimize relapses. In addition to these vast rehabilitation services, a designated Alzheimer’s unit offers specialized programs to address the challenges that affect the memory-impaired, with medical and caregiving professionals who are specially trained in providing that level of care.A STRONG FOUNDATION
